Sickness Certification:

For periods of 7 days or more, a GP will need to certify that you are not fit for work.
For ongoing conditions where further sick notes are needed, the request should be put in writing using the forms provided in the reception foyer.
We will contact you within 7 days  to advise when this is ready for
collection. A doctors sickness certification is not required for periods of less than 7 days.  A self certification form which can be collected from reception.

Test Results:

 

If you have had any tests or x-rays undertaken or ordered by the practice, a GP will contact you by telephone, email or in writing regarding any results needing review.
The quickest way to track and view any test results is to register to use our online services. 
Our reception staff are not medically trained to review or provide any results.

Chaperones:

 

If you would like a Chaperone present during a physical examination with one of our clinicians please let one of the reception team or the GP know. At times the GP may feel it is essential for a Chaperone to be present during an examination.

Book an Interpreter:

To ensure we are able to provide equal access to all patients, we are able to book an interpreter or translation services for your appointment. We are able to provide this service for patients whose first language is not English or for patients requiring sign language due to a hearing impairment.
